Nomination Process
Submit the nomination form here
There are two optional questions at the end of the form. One, to add supporting materials to the nominee’s profile such as photos, other awards they've received, news stories about their work, etc. and two, to add a letter of support from another community member, organization, or other person who has witnessed the nominee’s positive impact. These are optional but may help strengthen the nominee's profile for consideration. We recommend gathering these items before proceeding to fill out the nomination form.
Note that all nominations are confidential: we ask that you keep your intention to nominate a Citizen private.
